Men det som ER interessant med den metaanalysen er faktisk at den setter baseline ratio mellom t4 og t3 for eutyroide til 4,3 .... (tabell 3) noe som er overraskende likt Erfa sin.
Det er også mye interessant stoff i kapitlet "Is There an Unmet Need in L-T4-Treated Hypothyroid Patients?"
Så selv om konklusjonen av metaanalysen er nedslående - så åpner den iallfall for å skyte i filler påstanden om at t4 tilfredsstiller alle kroppens behov. Det finnes faktisk bevis i fleng i analysen...The study provided evidence that patients on L-T4 replacement even with a normal serum TSH display significant impairment of psychological well-being compared to controls of similar age and sex. Serum-free T4 (but not free T3) showed a negative relation to GHQ-12 scores (higher free T4 was associated with lower GHQ-12 scores implying improved well-being), whereas a positive correlation was seen with TSH and GHQ [12].

Another community-based study from The Netherlands investigated 141 patients who had been on L-T4 replacement for primary autoimmune hypothyroidism for at least 6 months and had serum TSH values between 0.11 and 4.0 mU/l [13]. Results of neurocognitive tests (cognitive or psychomotor speed, attention, working memory as well as learning and memory) and scores on questionnaires (Symptom Check List-90 total score and the Rand 36-item Health Survey subscales for mental health and vitality) of patients were compared with reference values of these tests for the Dutch general population. Patients showed poor performance on various domains of neurocognitive functioning compared with standard reference values (p = 0.001), especially on a complex attention task and on verbal memory tests. Levels of well-being were significantly lower for patients compared with those of the general population (p = 0.001). Neither serum TSH nor anti-thyroid antibodies were determinants of neurocognitive functioning or well-being. The results of this study suggest that neurocognitive functioning as well as psychological well-being may not be completely restored in patients with hypothyroidism despite L-T4 treatment.

A third community-based study from Norway among subjects aged ≥40 years compared HADS scores of 1,546 women on L-T4 and 18,137 women without thyroid disease not on L-T4 [14]. Serum TSH (median values with interquartile range) in both groups were 1.25 (0.3–2.9) and 1.5 (1.1–2.1) mU/l, respectively. More women on L-T4 had high scores (HADS ≥8) for depression and anxiety than women not on L-T4: depression 18.4 versus 12.8% (p < 0.001), anxiety 23.4 versus 18.6% (p < 0.001), respectively. In women on L-T4, serum TSH was positively associated with depression and anxiety. For each 1 mU/l increase in serum TSH there was 3% more depression cases (p = 0.04) and 3% more anxiety cases (p = 0.046). In women not on L-T4, no significant associations with depression or anxiety were seen. The study suggested a higher prevalence of depression and anxiety in L-T4-treated hypothyroid women of 40 years and older.Sist endret av smgj; 27-06-13 kl 07:23
