Treating an underlying thyroid problem is critical to alleviating the associated psychiatric symptoms. The first hurdle to treating underlying hypothyroidism is in its diagnosis. Hypothyroidism is a medical condition commonly encountered in a variety of clinical settings. The clinical presentations of thyroid hormone deficiency are diverse, complicated, and often overlooked. Hypothyroidism is a potential etiology for multiple somatic complaints and a variety of psychological disturbances. The physical complaints are primarily related to metabolic slowing secondary to lack of thyroid hormone. Psychiatric presentations include cognitive dysfunction, affective disorders, and psychosis. The realization that hypothyroidism might be the potential etiology of an assortment of symptoms is critical in the identification and treatment of the hypothyroid patient. Once hypothyroidism is identified, symptoms usually respond to appropriate thyroid hormone supplementation. This article presents a case of clinical hypothyroidism that came to clinical attention due to psychotic symptoms consisting of auditory and visual hallucinations. The case is followed by a brief discussion of the literature describing the relationship of hypothyroidism and psychiatric symptomatology.

One of the common symptoms of hypothyroidism is a depressed mood. However, most patients with major depression are biochemically euthyroid. On closer examination, a number of subtle abnormalities in the hypothalamus-pituitary-thyroid (HPT) axis occur in a large proportion of depressed patients, including alterations in serum concentrations of thyroid hormones and TSH. In addition, a number of clinical studies have suggested a therapeutic role for thyroid hormone co-medication in patients with depression who are biochemically euthyroid. The lack of insight into the pathogenesis of the neuroendocrine changes in the HPT axis, together with the uncertainty about a possible role for thyroid hormone treatment in depression have made the field somewhat controversial. In this paper, the need for larger and well-designed clinical studies on a possible role for T3 in the treatment of depression is discussed, while a hypothesis is put forward on the pathogenesis of HPT axis changes in major depression.

The psychiatric disturbances which accompany hyperthyroidism and hypothyroidism, the two commonest thyroid disorders, mimic mental illness. People with an overactive thyroid may exhibit marked anxiety and tension, emotional lability, impatience and irritability, distractible overactivity, exaggerated sensitivity to noise, and fluctuating depression with sadness and problems with sleep and the appetite. In extreme cases, they may appear schizophrenic, losing touch with reality and becoming delirious or hallucinating. An underactive thyroid can lead to progressive loss of interest and initiative, slowing of mental processes, poor memory for recent events, fading of the personality's colour and vivacity, general intellectual deterioration, depression with a paranoid flavour, and eventually, if not checked, to dementia and permanent harmful effects on the brain. In instances of each condition, some persons have been wrongly diagnosed, hospitalized for months, and treated unsuccessfully for psychosis.

A huge industry has arisen around the treatment of depressive illness and psychiatrists are gainfully employed in their thousands. Whether it is more widespread than it was is perhaps difficult to answer. There are more of us to be depressed; we have more to be depressed about, and we are more likely to seek help. But there certainly seems to be more people troubled by depression and the great panoply of antidepressant medication tells its own story.

Before having a look at thyroid deficiency and its link to depression, we should learn a bit about it, and how it is caused and why. People who are depressed are sad, unmotivated most of the day and are usually worse in the morning. They sleep poorly, and wake up tired; they feel worthless, they have a poor self-image.

Background: To evaluate the association between mood and anxiety disorders and thyroid autoimmunity in a community sample. Methods: A community based sample of 222 subjects was examined. Psychiatric diagnoses were formulated using the International Composite Diagnostic Interview Simplified (CIDIS), according to DSM-IV criteria. All subjects underwent a complete thyroid evaluation including physical examination, thyroid echography and measure of serum free T4 (FT4), free T3 (FT3), thyroid-stimulating hormone (TSH) and anti-thyroid peroxidase autoantibodies (anti-TPO).

Results: 16.6% of the overall sample had an anti-TPO value above the normal cut-off. Subjects with at least one diagnosis of anxiety disorders (OR = 4.2, C.L. 95% 1.9–38.8) or mood disorders (OR = 2.9, Cl 95% 1.4–6.6, P < 0.011) were positive for serum anti-TPO more frequently than subjects without mood or anxiety disorders. A statistically significant association with anti-TPO+ was found in Anxiety Disorder Not Otherwise Specified (OR = 4.0, CL 95% 1.1–15.5), in Major Depressive Episode (OR = 2.7, CL 95% 1.1–6.7) and Depressive Disorder Not Otherwise Specified (OR = 4.4, S CL 95% 1–19.3).

Conclusions: The study seems to suggest that individuals in the community with thyroid autoimmunity may be at high risk for mood and anxiety disorders. The psychiatric disorders and the autoimmune reaction seem to be rooted in a same (and not easy correctable) aberrancy in the immuno-endocrine system. Should our results be confirmed, the findings may be of great interest for future preventive and case finding projects.

OBJECTIVE: The delayed onset of therapeutic response to antidepressants remains a major problem in the treatment of depression. Among the strategies to accelerate response to treatment, the early addition of thyroid hormone to antidepressants has been suggested as a viable method. The authors performed a meta-analysis of the literature on the use of thyroid hormone supplementation to accelerate the treatment of depression to determine whether there is sufficient evidence to support the clinical efficacy of this strategy. METHOD: Both a computer-aided search of the National Library of Medicine MEDLINE and an intensive search by hand were conducted to identify all double-blind, placebo-controlled studies assessing the concomitant administration of thyroid hormone and antidepressant to accelerate clinical response in patients with nonrefractory depression. RESULTS: Six studies were identified. All were conducted with triiodothyronine (T3) and a tricyclic antidepressant. Five of the six studies found T3 to be significantly more effective than placebo in accelerating clinical response. The pooled, weighted effect size index was 0.58, and the average effect was highly significant. Further, the effects of T3 acceleration were greater as the percentage of women participating in the study increased. CONCLUSIONS: This meta-analysis supports the efficacy of T3 in accelerating clinical response to tricyclic antidepressants in patients with nonrefractory depression. Furthermore, women may be more likely than men to benefit from this intervention.

Another study finds an association between an abnormal thyroid condition and bipolar disorder, pointing to the possibility that a simple blood test could help identify those at risk.

Combining low dose T3 and low dose T4 seemed to help one young patient very much. After her response I used the same approach for about 25 patients who were in some ways similar to her. I think it helped quite a few of them too, but this is not a good way to arrive at any conclusions about whether a treatment "works" (there is too much risk of hope biasing the results, both in the patients and in the clinician).

Several studies have underlined the high prevalence of psychiatric symptoms and disorders in thyroid diseases. The aim of this study was to evaluate the prevalence of psychiatric disorders in 93 inpatients affected by different thyroid diseases during their lifetimes, by means of a standardized instrument, i.e., the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders-III-Revised, Upjohn Version (SCID-UP-R). The results showed higher rates of panic disorder, simple phobia, obsessive-compulsive disorder, major depressive disorder, bipolar disorder and cyclothymia in thyroid patients than in the general population. These findings would suggest that the co-occurrence of psychiatric and thyroid diseases may be the result of common biochemical abnormalities.
