tak for dine råd - det var netop det jeg var ude efter - m.h.t lægerne er " slås kampen igang " derfor bruger jeg nogle af mine få kræfter på at få viden - allerede nu ved jeg England har forsøgt med plasma ( ala dialyse men uden sønderlig succes )

Jeg får en kontakt læge så må jeg se om han er noget værd ellers er det ud i verden - det må jeg jo kunne få støtte til når der ikke er viden her hjemme - ellers flytter jeg hen hvor viden er.

Til Finn - stress er jo fjenden for stofskifte patienter - idette tilfælde mere end du aner-. kunsten er balance hvilket jeg p.t kæmper for at opnå uden at miste " et mål for at vide ligeså meget som lægerne ". Allerede nu har deres manglende viden gjort nogle missere ". der er dog en ung læge i mit ringhjørne som hvis han kan bevare sig selv har potientiale til at blive rigtig god.

Hashimoto's encephalopati. Jeg er nydiagnostiseret - anti på 75000 og var væk i 4 døgn er nu i prednison behandling og de er faldet til 23000

Det er rigtigt at der er lang imellem gode kilder til viden om Hashimoto's encephalopati. Een jeg er faldet over i dag Brain perfusion abnormalities in patients with euthyroid autoimmune thyroiditis fastslår en langt højere end hidtil antaget fællesnævner mellem Hashimoto's encephalopathy og den "almindelige" Hashimoto's thyroiditis. Det lyder ikke godt, særlig når lægerne har for (u)vane at "vente og se", hvis de mener at patienternes antistoffer ikke er "høje nok" eller at patienternes øvrige thyreoidea-blodprøver er for "fine" til at iværksætte behandling.

Håber den unge læge er fortsat i dit ringhjørne og at det er lykkedes dig at komme i kontakt med dygtige specialister.

Abscract

Patients with Hashimoto's encephalopathy (HE), a steroid-responsive disorder, associated with Hashimoto's disease and high levels of thyroid-related autoantibodies usually present with a subacute onset of confusion, focal or generalized seizures. Frequent EEG abnormalities include generalized, rhythmic bifrontal or temporal slowing. Elevated protein levels or an intrathecal IgG synthesis may be present in cerebrospinal fluid (CSF). A 39-year-old woman underwent a relapsing course of myocloni and generalized seizures. Initially, thyroid function, thyroid-related autoantibody screening and cerebral MRI were unrevealing. CSF showed oligoclonal bands. Short-term treatment with high doses of prednisolone resolved the myocloni. During the 5th episode of myocloni, signs of hyperthyroidism and elevation of thyroid microsomal antibody titer developed. Hashimoto's thyroiditis and HE were diagnosed. After subtotal thyroidectomy the patient remained asymptomatic.

Abstract

Abstract: Three cases are described of a reversible encephalopathy, all presenting with marked neurological disturbance. In all three, the diagnosis was not clear at the time of presentation but eventually it was felt all of the cases were consistent with Hashimoto’s encephalopathy. The diagnosis of Hashimoto’s encephalopathy should be considered in any case of unexplained encephalopathy. Common features are high anti-thyroid peroxidase antibody titres, an abnormal EEG and an elevated CSF protein concentration. The encephalopathy is independent of thyroid hormonal status. Treatment with corticosteroids leads to a prompt resolution of symptoms and long term low dose steroid therapy prevents further neurological recurrence Key words: Hashimoto’s, acute, emergency, encephalopathy, medical.
