    My Recommendations

    Adult patients no longer tolerate the one-grain starting dose of desiccated thyroid that Dr. Barnes recommended. A growing percentage of patients may require therapy for the environmental toxins they harbor prior to beginning thyroid. Toxic chemicals or heavy metals like mercury may block the entire cascade of chemical reactions involved in thyroid metabolism as well as other hormones. The resultant compromised endocrine, nervous, and immune system contribute to and exacerbate Type 2 hypothyroidism.

    Most adult patients tolerate one-quarter to one-half of a grain. For safety’s sake I recommend beginning with one-quarter grain. It is often difficult to ascertain who will be intolerant of the medication. In general, the healthier the patient is, the more likely they will tolerate thyroid. Patients with severe allergies, chemical sensitivities, and chronic pain tend to be less tolerant of desiccated thyroid.

    These guidelines also apply to Type 1 hypothyroid patients already taking T4 (Levoxyl, Synthroid). I treat Type 1 patients on T4 by slowly adding desiccated thyroid, and do not stop the T4 until they are tolerating desiccated thyroid and improving. The stopping og T4 is also gradual. The patients who are intolerant of desiccated thyroid may tolerate T4 . Their symptoms often improve by slowly increasing the dosage to tolerance.

    For the majority who do tolerate one-quarter grain, I increase the dosage one-quarter grain every 7 to 14 days up to one grain, depending upon the constellation of symptoms, age, and condition on the patient’s heart. Gradual increases continue at about one-half grain per month depending on the patient’s response. When patients remain symptomatic and show no evidence of side effects, their dosage is increased to four grains before waiting several months in advance of further increase.

    My patients require an average dose age of three to three and one-half grains, the same dosage the Belgian study required. Some adult patients may only need two grains while others need five. Dr. Sonkin stated in his literature that patients may require as much as 6 to 12 months before responding to thyroid replacement.

    There is no no substitute for clinical experience combined with common sense.
